1. Introduction

These Terms and Conditions (“Terms”) set out the basis on which Jo Condon Autism Assessments (“we,” “us,” “our”) provides private autism assessment, ADHD assessment and related services (“Services”) to clients (“you,” “your”).

By booking, attending, or paying for any Service, you agree to be bound by these Terms. If you do not agree, please do not proceed with our Services.

2. Nature of services

Our Services include diagnostic assessments for autism spectrum disorder and attention deficit hyperactivity disorder (ADHD) and may include related consultations, feedback sessions, and written reports.

Assessments are carried out by qualified professionals in accordance with current clinical standards and professional guidelines (for example, the NICE Guidelines and DSM-5 criteria).

Our Services are for diagnostic and advisory purposes only and do not constitute medical treatment or emergency care.

3. Booking and eligibility

  • You must be aged 18 or over to book an assessment for yourself, or you must be the parent or legal guardian if booking for a minor.
  • We require background information before confirming an appointment.
  • We reserve the right to decline a referral if the Service requested is outside our clinical scope or inappropriate for the client’s needs.

4. Consent

  • Clients over the age of 18 must consent to the assessment and sharing of information themselves.
  • We recognise that some individuals aged 18 or over may find arranging an assessment difficult. You may nominate a representative to liaise with us on your behalf. We will need signed consent from you to liaise with a representative; this may be withdrawn at any time.
  • We require consent from a person with parental responsibility or legal guardianship to consent to the assessment of anyone aged under 18.
  • Where parents are separated or co-parenting, we need the consent of one parent to complete the assessment. It is the responsibility of the person commissioning and consenting to the assessment to share information regarding the assessment with any other person who holds parental responsibility. We do not require consent from both parents and will presume that both parents are in agreement with the assessment. If, at any time, we become aware that the other parent is not consenting to the assessment, we will stop the process. The costs for work completed will not be refunded.   • Persons with parental responsibility are entitled to request and receive information that we hold about the child, including the diagnostic report, regardless of who commissioned the assessment.

5. Fees, invoicing and payment

  • Fees for assessments are available on our website
  • Full or part payment must be made prior to the assessment.
  • If choosing part payment option, the second payment must be made no more than 3 months after the first payment.
  • Reports will only be released once full payment has been received

6. Cancellation

  • Client cancellations:
    • Appointments cancelled with less than 48 hours’ notice may be charged in full
    • Refunds for cancelled assessments will be subject to the stage in the process at which cancellation is requested
  • Clinic cancellations:
    • If we need to reschedule or cancel, we will notify you as soon as possible and offer an alternative date
  • Refunds will be processed via the original payment method within a reasonable timeframe.

7. Confidentiality and data protection

We are committed to protecting your privacy and confidentiality.

  • All personal information is handled in accordance with the UK GDPR
  • Information you provide is used solely for clinical, administrative, and legal purposes.
  • We will not share your information with third parties without your consent, except:
    • where disclosure is required by law, or
    • where there is an imminent risk of harm to yourself or others.
  • You have the right to access, rectify, or request deletion of your personal data.

8. Your clinicians

Your assessment will be completed by appropriately qualified and experienced clinicians. Your clinician will:

  • Be registered with their relevant professional body (e.g. HCPC, NMC, GMC)
  • Have completed post-qualification training in the diagnostic assessment of neurodevelopmental disorders
  • Hold a clear, enhanced DBS certificate
  • Have regular clinical supervision
  • Be experienced in assessing and supporting the broad range of neurodevelopmental conditions and presentations across the age range

We are unable to provide details of your clinician prior to the appointment session being arranged, but you will be advised of your clinician at the time of appointment booking.

9. Assessment process

Our assessment processes follow the national standards outlined within the relevant NICE guidelines.

Autism assessment

Background information forms: At the beginning of the autism assessment process, you will receive links to background information forms and questionnaires. These will need to be completed before the assessment sessions can be booked. If you are having difficulties completing these forms, our team will offer assistance. Delays in completing the forms will delay the assessment process.

Education setting consent (for children attending an education setting): A robust assessment process includes gathering information about how the child manages in their education setting. To obtain this information, we require written consent from a person with parental responsibility. You are not obliged to provide consent and do not have to consent to the sharing of information between us and your child’s education setting. Please note that if you do not consent to us seeking information from the setting, the quality of the assessment may be impacted. Any delay in providing consent or in the school providing the information required may delay the outcome of the assessment.

Observation session: The observation session will be arranged for a mutually convenient time. Please be aware that we try to accommodate individual circumstances, but we cannot guarantee being able to offer sessions outside of typical weekday office hours, and you may need to attend appointments between 9 and 5 on Monday – Friday.

For children under the age of 10, the observation must be completed face-to-face at one of our clinics. For those aged 10 and over, the observation session may be completed at our clinics or online.

Online observations are complete via the MS Teams platform.

Online observation session must be completed using a tablet, laptop or PC. We cannot complete an observation on a mobile phone screen.

It is your responsibility to ensure that you have the correct equipment and internet connection.

Diagnostic history session: Depending on the age and circumstances of the person being assessed, the diagnostic history session may be completed with the person being assessed or a parent/informant. The clinical team will discuss the best option with you.

Diagnostic history sessions are completed online via the MS Teams platform or via the telephone. We do not offer these sessions in person. The session will be arranged for a mutually convenient time. Please be aware that we try to accommodate individual circumstances, but we cannot guarantee being able to offer sessions outside of typical weekday office hours and you may need to attend appointments between 9 and 5 on Monday – Friday.

Report: Your report will be password-protected and sent via email.

Feedback session: The feedback session is optional; you will be asked whether you want to arrange this session. Feedback sessions are completed online via the MS Teams platform or via the telephone. We do not offer these sessions in person. The session will be arranged for a mutually convenient time.

ADHD Assessment

Background information forms: At the beginning of the ADHD assessment process you will receive links to background information forms and questionnaires. These will need to be completed before the assessment session can be booked. If you are having difficulties completing these forms, our team will offer assistance. Delays in completing the forms will delay the assessment process.

Education setting consent (for children attending an education setting): A robust assessment process includes gathering information about how the child manages in their education setting. To obtain this information, we require written consent from a person with parental responsibility. We are unable to complete an ADHD assessment without being able to obtain information from the education setting.

Diagnostic interview: Depending on the person being assessed, the diagnostic interview may be completed with the person being assessed or a parent / informant.

Diagnostic interviews are completed online via the MS Teams platform. We do not offer these sessions in person or via the telephone. The session will be arranged for a mutually convenient time. Please be aware that we try to accommodate individual circumstances, but we cannot guarantee being able to offer sessions outside of typical weekday office hours and you may need to attend appointments between 9 and 5 on Monday – Friday.

Child session: A separate session with the child being assessed may be required, depending on the nature of the assessment. If the clinical team request this, the session will be delivered via MS Teams and will be arranged for a mutually convenient time. Please be aware that we try to accommodate individual circumstances, but we cannot guarantee being able to offer sessions outside of typical weekday office hours, and you may need to attend appointments between 9 and 5 on Monday – Friday.

Report: Your report will be password-protected and sent via email.

Feedback session: The feedback session is optional; you will be asked whether you want to arrange this session. Feedback sessions are completed online via the MS Teams platform or via the telephone. We do not offer these sessions in person. The session will be arranged for a mutually convenient time.

10. Your report

We provide robust diagnostic reports which clearly outline the information gathered during the assessment and the diagnostic outcome.

Please note that we will only release completed reports after MDT discussion and the QA process. We will not release a report before the full process has been completed and we do not provide reports for partly completed assessments.

Autism assessment

Your report will contain the following information:

  • Outcome of assessment
  • DSM 5 criteria met
  • Current concerns
  • Family history
  • Early developmental history
  • Medical history
  • Educational history
  • Information provided by school (where applicable)
  • Employment history (where applicable)
  • Information gathered during the developmental history interview
  • Observations noted during the ADOS
  • Any other relevant information
  • Recommendations

ADHD assessment

Your report will contain the following information:

  • Outcome of assessment
  • DSM 5 criteria met
  • Current concerns
  • Family history
  • Early developmental history
  • Medical history
  • Educational history
  • Information provided by school (where applicable)
  • Employment history (where applicable)
  • Information gathered during the diagnostic interview
  • Observations noted during child session (if applicable)
  • Any other relevant information
  • Recommendations

If there is an error in your report as a result of our misunderstanding something that you shared with us, or a misspelling of something you shared with us, we will be more than happy to correct this and reissue your report. We will not be able to make changes that are not reflective of the assessment sessions or other information provided.

11. Client Responsibilities

You agree to:

  • Provide complete and accurate information relevant to the assessment;
  • Attend scheduled appointments on time;
  • Follow the instructions and procedures necessary for a valid assessment;
  • Respect the professional boundaries of our clinicians and staff.
  • Where applicable, keep any other person with parental responsibility informed of the assessment process and outcome

Failure to cooperate or provide truthful information may affect the accuracy and validity of the assessment outcome.

12. Clinical Limitations and Disclaimer

  • While every effort is made to ensure professional accuracy, assessments are based on the information and presentation available at the time.
  • Diagnoses are clinical opinions, not absolute determinations.
  • Our Services are not substitutes for ongoing medical or psychological care.
  • Our assessments are NICE compliant and readily accepted by the NHS, schools and other public bodies. However, we make no guarantee that external bodies (e.g., NHS, schools) will accept or act upon our reports.

13. Liability

To the fullest extent permitted by law:

  • We exclude all liability for indirect, incidental, or consequential loss arising from the use of our Services.
  • Our total liability for any claim, whether in contract, negligence, or otherwise, shall not exceed the amount paid for the relevant Service.
  • Nothing in these Terms limits or excludes liability for death or personal injury caused by negligence, or for fraud or fraudulent misrepresentation.

14. Complaints Procedure

Please refer to our complaints policy.

15. Amendments to Terms

We may update these Terms periodically. Updated versions will be published on our website, and the revised effective date will appear at the top of this document. Continued use of our Services after changes take effect constitutes acceptance of the revised Terms.

16. Governing Law

These Terms shall be governed by and construed in accordance with the laws of England and Wales.

Any disputes shall be subject to the exclusive jurisdiction of the courts of England and Wales.
